Treatment & Management of Low Back Pain

“Worldwide, low back pain causes more disability than any other condition”.¹

The National Institute for Health and Care Excellence defines low back pain as soreness or stiffness in the back, between the bottom of your rib cage and the top of your legs.¹

Other symptoms that may accompany low back pain include sciatica. Sciatica is a word that is used to describe leg pain that is secondary to a nerve (nerve root or spinal nerve) being effected.¹

Episodes of back pain usually have improvements in pain and disability within a few weeks to a few months with primary care management.  The need for investigations or referral to specialist services will be considered if the result is likely to change management.¹

Managing modifiable risk factors for low back pain can reduce the risk of developing a chronic condition. These risk factors include:

Insufficient physical activity

Smoking

Overweight/obesity

Occupational hazards

Joint trauma and injuries.²

Non-pharmacological interventions include self-management (education and advice), exercise, and manual therapy.¹

People’s specific needs, preferences and capabilities should be taken into account with the application of these therapies.¹

Are you experiencing low back pain? We may be able to help. Speak to one of our Osteopaths at Valued Health Osteopathy in Bentleigh East about your treatment and rehabilitation options. Call or book online today.

Treatment & Management of Plantar Fasciitis

Plantar fasciitis is a common cause of foot pain, affecting 3.6-7% of the general population and accounting for 8% of all running-related injuries.¹ It is characterized by pain on the under-surface of the foot around the heel and is typically provoked by weight-bearing activities. It is prevalent in both sports active and sedentary population groups.¹

Research has suggested that high-load strength training intervention enables a quicker reduction of pain.¹ This includes inducing high-load tensile forces across the plantar fascia by combining the use of achilles loading and the windlass mechanism. The windlass mechanism is the tension that is created on the under-surface of the foot between the heel and the ball of the foot when the big toe is extended. This can be incorporated into a strength training intervention by using a towel under the toes when performing a heel raise to increase the amount of extension of the big toe.

Although previous studies have shown that plantar fascia-specific stretching and shoe inserts are effective treatment, approximately 40% of sufferers continue to experience symptoms 2 years after a diagnosis had been made.¹ Therefore a simple progressive exercise protocol should be included in the treatment and management of plantar fasciitis.

Scoliosis & Osteopathy

Scoliosis & Osteopathy | Find out how we can help?

Dr. Sarah Walsh (Osteopath) and Schroth Method Certified Scoliosis practitioner.

Scoliosis is a condition in which the spine deviates in all three planes of motion (1). It can be observed as an asymmetrical silhouette of the trunk when standing, and more apparent when bending over. Visible asymmetries may be present such as; one shoulder sitting higher than the other, the trunk may shift to one side, one hip may appear more prominent than the other, or a prominence of the ribs on one side.

To assess the degree of the curve, a measurement is taken on an x-ray from the upper end vertebrae and lower end vertebrae in the frontal plane, this is called the Cobb angle. The Scoliosis Research Society suggests that scoliosis is defined as a Cobb angle that is greater than 10 degrees, although structural scoliosis can be seen with less than this with a potential for progression (1).

Approximately 80% of scoliosis cases are idiopathic scoliosis (1), meaning it is not possible to find a specific disease causing the deformity. The remaining 20% of scoliosis cases are secondary to another pathological process such as neurological and congenital causes, therefore an extensive case history, examination and radiology is required. Although most scoliosis cases are idiopathic, when untreated it may lead to severe trunk deformities. This may limit the capacity and functional biomechanics of the chest, exercise capacity, general fitness and ability to work (1).

Idiopathic scoliosis commonly appears between 6 and 24 months, between the age of 5 and 8 years, and between 11 to 14 years during periods of rapid growth (1). There is a much lower potential for progression of the curve after spinal growth is complete. However, in adults, idiopathic scoliosis may intensify with degenerative change due to uneven loading of the spine.

The aim of scoliosis management is to halt curve progression, improve lung function, manage pain associated with the scoliosis, and improve postural appearance. Treatment options currently include observation, scoliosis specific exercise, bracing, and surgery. Bracing involves the use of a corrective orthosis worn for a specified period each day, usually worn until maturity, with the goal of halting curve progression. While bracing is the primary treatment approach during phases of intense growth, specific rehabilitation during all phases is recommended (2).

The Schroth Method is an exercise program that is aimed at treating scoliosis in adults and adolescents. The Schroth Method was developed by Katharina Schroth in the 1920s when she herself, a scoliosis sufferer, was inspired by a balloon to breathe away the deformities of her own trunk. The Schroth Method consists of specific postural correction, correction of breathing patterns, and correction of postural perception.

Each scoliosis case is treated on an individual basis and requires thorough clinical evaluation and patient history. Exercises for scoliosis should not simply consist of general exercises but exercises specific to the scoliosis to address the spinal deformity (3).

Can foam rolling help with your recovery?

Foam rolling is a form of self-myofascial release or self-manual therapy. It is often used with the aim of improving flexibility 🤸🏻‍♂️, recovery, and athletic performance. It is intended to reduce localised tightness 💢 by influencing the myofascial complex. The word ‘myo’ referring to the Latin term for muscle and ‘fascia’ being the soft-tissue component of the connective tissue system that permeates the human body, forming a continuous, whole-body, three-dimensional matrix 🌐 of structural support.1

Restrictions in joint range of motion and local blood flow are thought to be attributed to localised tightness of the myofascial complex and that manual manipulation 👐 of this tension will allow the tissue to become softer and more pliable.2 Research suggests that fascia contains mechanoreceptors. When using the foam roller, it is thought that the pressure applied subsequently stimulates these mechanoreceptors, influencing the central nervous 🧠 system. Therefore, altering the activity ⚡️ of the muscle and reducing localised myofascial tightness.3

Research suggests that foam rolling may improve short-term flexibility and has no negative impact on athletic ⛹️‍♂️ performance.4 Research also suggests that foam rolling may even have a long-term effect on flexibility when performed on a regular basis.5 It is also believed to reduce the sensation of delayed onset muscle soreness (DOMS) following exercise, possibly allowing the recovery ♨️ process post-exercise to be sped up.6

Foam rolling is typically used during warm-up 🔥 and cool down ❄️ routines to prepare for physical activity and attempt to reduce the effects of DOMS and promote recovery. A possible protocol could include 3-5 sets of 20-30 second repetitions, performed 3-5 times per week

👇 Shown here are some treatment techniques and rehabilitation exercises that we find helpful with treating Tennis Elbow 👇

1️⃣ Resisted forearm pronation: Hold the band taut in your hands. The band should wrap around the back of your hand, and around the outside to your palm. Turn your affected hand so your palm faces the floor, stretching the band. Control the movement as you turn your palm to face the ceiling again.

2️⃣ Resisted forearm supination: Hold a resistance band taut in your hands. Turn your affected hand so your palm faces the ceiling, stretching the band. Control the movement as you return your palm to face the floor again.

3️⃣ Active supination/pronation with dumbbell: Hold a dumbbell in your hand and out over the edge of the table. Rotate the palm of your hand so it turns up and down, controlling the momentum and rotation of the dumbbell.

4️⃣ Eccentric wrist extension with weight: Start with holding a weight in your hand with your palm facing downwards. Using the other hand, lift your wrist up, keeping your forearm in contact with the table. Remove your other hand and then slowly lower your raised hand back down without support. Relax and repeat.

✅ Try 1-2 sets of 10-15 reps aiming to build up to 20 reps. Use a light theraband and light weight to start with.

😡 If you have elbow pain or think you have tennis elbow symptoms, we recommend you consult with your Osteopath or health professional to find out the “do’s and don’ts” depending on the severity of your injury as there is no ‘one size fits all’ with treatment plans.
