Treatment & Management of Plantar Fasciitis

Plantar fasciitis is a common cause of foot pain, affecting 3.6-7% of the general population and accounting for 8% of all running-related injuries.¹ It is characterized by pain on the under-surface of the foot around the heel and is typically provoked by weight-bearing activities. It is prevalent in both sports active and sedentary population groups.¹

Research has suggested that high-load strength training intervention enables a quicker reduction of pain.¹ This includes inducing high-load tensile forces across the plantar fascia by combining the use of achilles loading and the windlass mechanism. The windlass mechanism is the tension that is created on the under-surface of the foot between the heel and the ball of the foot when the big toe is extended. This can be incorporated into a strength training intervention by using a towel under the toes when performing a heel raise to increase the amount of extension of the big toe.

Although previous studies have shown that plantar fascia-specific stretching and shoe inserts are effective treatment, approximately 40% of sufferers continue to experience symptoms 2 years after a diagnosis had been made.¹ Therefore a simple progressive exercise protocol should be included in the treatment and management of plantar fasciitis.
