Treatment & Management of Soft Tissue Injuries

Soft tissue injuries no longer need “RICE” but instead need “PEACE & LOVE”. The British Journal of Sports Medicine recently posted a blog on the development of a new acronym for the management of soft tissue injuries that addresses both immediate care as well as subsequent management.

For immediately after a soft tissue injury, let PEACE guide your approach. Once the first days have passed, soft tissue injuries need LOVE.

P for Protect – Unload or restrict movement for 1 to 3 days to avoid activities that increase pain.

E for Elevate – Elevate the injured limb higher than the heart to promote fluid flow.

A for Avoid anti-inflammatory modalities – Avoid taking anti-inflammatory medications and avoid icing as it may reduce tissue healing.

C for Compress – Compression with taping or bandages help reduces swelling.

E for Education – Education on the condition, load management and the benefits of an active approach to recovery will help avoid overtreatment.

&

L for Load – Loading and mechanical stress should be added early and normal activities as soon as your symptoms allow.

O for Optimism – Optimism enhances the likelihood of an optimal recovery as catastrophisation, depression and fear can represent barriers to recovery.

V for Vasularisation – Cardiovascular activity started a few days after injury boosts motivation and increases blood flow to the injured site.

E for Exercise – Exercises, with pain as a guide, helps to restore mobility, strength and proprioception after injury.

Treatment & Management of Low Back Pain

“Worldwide, low back pain causes more disability than any other condition”.¹

The National Institute for Health and Care Excellence defines low back pain as soreness or stiffness in the back, between the bottom of your rib cage and the top of your legs.¹

Other symptoms that may accompany low back pain include sciatica. Sciatica is a word that is used to describe leg pain that is secondary to a nerve (nerve root or spinal nerve) being effected.¹

Episodes of back pain usually have improvements in pain and disability within a few weeks to a few months with primary care management.  The need for investigations or referral to specialist services will be considered if the result is likely to change management.¹

Managing modifiable risk factors for low back pain can reduce the risk of developing a chronic condition. These risk factors include:

Insufficient physical activity

Smoking

Overweight/obesity

Occupational hazards

Joint trauma and injuries.²

Non-pharmacological interventions include self-management (education and advice), exercise, and manual therapy.¹

People’s specific needs, preferences and capabilities should be taken into account with the application of these therapies.¹
