What is Achilles Tendinopathy?


Heel pain? Ankle pain? Pain with running or walking? It could be Achilles Tendinopathy!

Achilles tendinopathy (previously known as achilles tendinitis) is a common condition in runners.¹


The achilles tendon is a thick band of tissue which arises from the soleus and gastrocnemius (calf muscles) and inserts at the calcaneus (back of the heel).² The achilles tendon is responsible for pulling on the heel of the foot to allow us to walk, run and jump.²


 Achilles tendinopathy is considered an overuse injury and can be caused by a sudden increase in the intensity, frequency or duration of an activity; or reduced recovery time between activity.²


Other risk factors for achilles tendinopathy include reduced plantar flexor strength, reduced ankle dorsiflexion and abnormal gait patterns.¹

Patients often present with pain localised to the achilles tendon, reduced range of motion and sometimes a thickened achilles tendon.² Stretching the achilles, running up hills or on a soft surface such as the beach may aggravate symptoms. This is due to the increase in ankle dorsiflexion which is where compression of the tendon is at its greatest.³


Initial treatment for Achilles tendinopathy should be aimed at reducing pain and improving calf strength.³ A rehabilitation program should then move towards a focus on the power of the whole kinetic chain and movement control during jumping and landing with retraining movements specific to the tendon in that sport/activity.³

WHAT IS TELEHEALTH?

Telehealth is a technology-based alternative to face to face consultations. It can include mediums such as video conferencing (E.G. Skype, Zoom, or many other platforms), internet or telephone.

All registered health practitioners can use Telehealth as long as it is safe and clinically appropriate for the health service being provided. Osteopaths must still comply with the same regulations of their national boards as when they provide face to face consultations.


TELEHEALTH AND WHAT IS INVOLVED

Firstly, your Osteopath will determine if a Telehealth consult will be appropriate for you and your concerns, if so, and you are happy to participate then we can organise a Telehealth appointment for you.

In these challenging times, IT security is of utmost importance. Before the consultation we ask for you to ensure you are within a safe and private area where you’re comfortable in having the appointment. We also utilise a Telehealth platform that provides security regarding broadcasting and connection. Your privacy and safety are of the upmost importance to us.

Via the video call we are able to have a chat to establish your presenting condition and discuss your past medical history. As we use video technology we may then ask you to do some different movements or exercises so we can evaluate your presenting complaint a little better. From there, we can discuss what we think may be occurring and develop a management plan that is tailored to you specifically. This may include some stretches (which we will be able to demonstrate), self-massage techniques (eg: spikey ball, foam roller etc), ergonomic and/ or postural advice. If it is appropriate, we can also create an individualised management and rehabilitation program for you to follow at home, this can be shared with you electronically after your consultation.

Dr Tom McCormick (Osteopath) believes it is important to address the immediate issues causing pain, however feels it is equally important to improve his patient’s ongoing movement, tissue quality, muscle activation and muscle strength in order to optimise injury recovery and reduce the chance of re-aggravation.


Since Tom graduated as an Osteopath he has had the opportunity to work both in Queensland and Victoria within private practice and has worked closely with sporting teams. Tom has suffered many injuries and complaints over the years due to his heavy interest in sport. From broken bones, torn cartilage, tendon and muscle injuries to even a double hernia repair, this is ultimately what led Tom into the health field.


Tom has a hands on approach to healthcare by utilising a broad range of techniques that are tailored specifically to each patient. Such techniques include mobilization, soft tissue massage, stretching, trigger point therapy, dry needling and exercise rehabilitation.

Returning To Exercise During COVID-19?

Wow, what a year 2020 has been so far! It has thrown a lot of challenges our way and we have all taken it in our stride and pushed on through. With so many recent changes, I’m sure we’re all looking forward to returning to our daily routines and for you, this may mean exercise.


As the restrictions start to ease throughout Australia, we now have the pleasure of starting to return to  a ‘new normality’ and getting back to doing more of what we love!

So what do you need to consider before returning to exercise during COVID-19?

  1. Firstly for many of us COVID-19 has meant isolation, restrictions, limited exercise and no gyms. As a result, we have most likely become more sedentary leaving our bodies feeling tighter, stiffer and not as strong. If this is the case, now is the time to pick ourselves up and get back into the swing of things.

2. When returning to exercise after a break it is important NOT to over-do it! Most people might believe that they can just jump straight back in and pick up where they left off. This may cause an injury or stir up previous niggles so it’s important to give your body time to adjust when returning to exercise.
3. Another helpful tip could be to slowly increase your activity levels and load over the first several weeks when returning to sporting activities or the gym. Some helpful examples might include increasing repetitions/sets and decreasing weight, increasing rest periods between sets and reps or simply reducing the time overall time you physically train for.
4. Lastly now is the perfect time to get back into your stretching, mobility and rehabilitation routine to help prevent injuries or niggles when returning to exercise or the gym. If you already have a routine, then that is great so go for it! Are you now working from home? For so many of us at the moment, things have changed very drastically, very quickly!

This in itself can be a lot to handle, so firstly take a deep breath and then give yourselves a pat on the back for adapting so well!


The current COVID-19 climate has meant that a lot of people have started working from home. This hopefully brings along a few benefits, such as getting to spend more time with family members, less commute time, less distraction and of course less exposure to COVID-19!


However, I’m sure this has raised many issues for people as well. It was implemented so quickly that I’m sure many people have been caught unaware, and it always takes a bit of time to feel things out and figure out your new normal. It might also have brought to light how well equipped our workplaces are and how lucky we are!

At home, we most likely aren’t going to be lucky enough to have all the ‘bells and whistles’ at our disposal for our workplace set up, but you have to work with what you’ve got!

Some KEY things to remember with your workplace set up include: 

  • Set up your screen so it is AT or 1/3 ABOVE your eye level.
  • Have the back rest slightly reclined 10-20 degrees and allow it to support your whole spine.
  • Keep the screen about an arm length from you.
  • Have your forearms approximately horizontal with your forearm support, knees at right angles and your thighs about horizontal with the ground (used a footrest if your feet don’t touch the ground).
  • Take a break for at least 1-2 minutes every 30 minutes of work and move your body however feels good! Without the distraction of working in an office, a lot of people seem to be getting caught up in their work. While its good to feel like you’re being productive, its possible that sitting for too long may lead to worse concentration levels as well as more musculoskeletal complaints.
  • Don’t forget to give your eyes a break too! Digital eye strain can lead to dry/irritated eyes, blurry vision, eye watering and headaches among other things. A good guide to follow for this is the 20-20-20 rule. Every 20 minutes, take a 20 second break to look at objects around 20 feet from you (that’s around 6 meters for us metric users).
  • Try to keep an ‘upright relaxed posture’ as discussed in our previous posture facts blog!   • If you had previous ergonomic adjustments made to your workstation at your workplace, make sure you continue with them at home! (e.g. the use of an ergonomic mouse or keyboard)
